Dr. (faculty supervisor) has been in touch with you concerning a position with the University of Washington, Department of ___________. It is a pleasure to be able to offer you a position as a Research Associate in (department), working on _________, for a ____-year period, beginning ________. You will receive a starting salary of $______ per month ($_______ annually). This salary includes any expected salary increases that might occur during the first year of your employment. You will be eligible for salary increases in subsequent years.
If mutually agreeable, reappointment for additional years will be considered during the last year of your appointment term.

Before your appointment recommendation can be acted upon formally, we must receive your written concurrence with the proposed terms of appointment as specified above. Please provide this as soon as possible, but in any case no later than _____. Enclosed are a University Biography Form and I-9 form. Please complete, sign, and return them with your letter of concurrence. Once you have obtained access to University computing systems you will be expected to complete the Affirmative Action Data Form and W-4 form online.

This will include, among other things, your birth date, social security number, and any other names by which you have been known.
A PhD degree is required to hold this title and must be conferred by the appointment start date. Evidence of conferral will be requested if the degree has been received within one year of the start date.
A condition of this appointment is that you resign any faculty appointment or other employment which you are holding elsewhere by the effective date of your University of Washington appointment.
