Candidates should also work closely with their departments to ensure that all departmental and college requirements are met.

- Student Teaching Evaluations
- Use the Teaching Chronicle template as the cover page and include all relevant quantitative and qualitative evaluations in reverse chronological order.
- Peer Teaching Evaluations
- Assistant Professor, Associate Professor Tenure Track, Professor Tenure Track, and Artists in Residence
- Include all peer evaluations since hire at UW - at least one per year in which teaching occurs
- Associate Professor
- Include all peer evaluations since hire or last promotion, whichever is more recent - required every three years
- A peer evaluation must be conducted in the academic year immediately preceding case submission to the provost’s office.
- Assistant Professor, Associate Professor Tenure Track, Professor Tenure Track, and Artists in Residence
- Outside Professional Work for Compensation (Form 1460), optional
